Hamburg home visit for senior citizens

Against the background of an aging society and the expected increase in people in need of help and care, from a health, care and socio-political point of view, the goal of a long, active and independent life in one's own four walls through early use of preventive potential and strengthening wins self-help skills will continue to gain in importance in the next few years.
The city of Hamburg introduced the Hamburg home visit for senior citizens in September 2018 as a new preventive, outreach, voluntary offer, initially in the districts of Eimsbüttel and Harburg. The first visits to these two districts will take place from October 2018. The Hamburg home visit is now to be introduced nationwide throughout Hamburg, which means that the Hamburg home visit will be carried out in all districts from the beginning of 2020. The aim of the Hamburg home visit is to promote active, independent living in one's own home in order to avoid loneliness in old age and to prevent or delay the onset of need for care.
The Hamburg home visit is a voluntary and free offer that does not address the occasion, but actively approaches older people ("walking structure") in order to provide information, advice and - if necessary and desired - contacts to existing professional services as well as offers of senior work to convey. As a rule, the Hamburg home visit consists of a one-time information and counseling session in your own home. It is expressly not a visiting service and does not replace professional advice from existing facilities such as the care support points. Rather, the aim of the Hamburg home visit is to identify prevention and support needs at an early stage and to facilitate access to the existing offers for older people in order to maintain quality of life and independence in old age and to support them to remain at home for as long as possible it is desired and can be made possible.
The topics of the Hamburg home visit can be varied, for example:
• health situation, diet and exercise,
• social integration and social contacts,
• engagement opportunities,
• Housing situation, especially accessibility and household help,
• any emerging care and support needs.
The department for "Hamburg home visits for senior citizens" has been commissioned by the authority for health and consumer protection to coordinate and carry out the Hamburg home visit. The Albertinen-Haus is responsible for the agency.
From 2020, the Hamburg home visit will be offered for senior citizens throughout Hamburg (so far only in Harburg and Eimsbüttel) to encourage active, independent living in their own home.
