You can use this application to apply for a disability assessment.

As a result of the examination, a degree of disability may be determined for you. If the examination of the required documents reveals a degree of disability of 50 or more, you can be issued with a severely disabled pass.

In the course of the examination, in addition to the degree of disability, any special health restrictions may be determined. The corresponding marks are entered in the severely handicapped ID card if necessary.

The following marks can be recognized by the competent authority and entitle you to further compensation for disadvantages:

G – Significant impairment of mobility in road traffic

aG – extraordinary walking disability

H - helplessness

B – entitlement to be accompanied at all times when using public transport

RF – Radio license fee reduction and/or telephone line fee reduction

GL - Deafness

BL - blindness

TBL - deaf-blindness

Important notes

Prerequisites

Your health has been so impaired for more than six months that it makes it difficult or impossible for you to participate in social life.
Your health impairments must be proven by medical documents.

Documents required

If possible, other evidence of the degree of disability (e.g. pension notice or corresponding administrative or court decision)
If possible, any medical documents in your possession (e.g. reports)
If necessary, power of attorney or supervisor ID
For non-EU applicants: Proof of your legal residence

Procedure

The determination is made upon request by the competent authority.

After you have submitted your application, any documents you may have submitted will be checked. If necessary, the competent authority will independently request further documents in order to clarify the medical facts.

All documents then available will be checked again. After your documents have been checked, you will receive a notification from the responsible office, which will determine your degree of disability and any characteristics.

Processing time

The processing time is individual and depends, among other things, on the completeness of the information and the medical documents to be requested.
