Prerequisites

Professional license as a pharmacist and further training qualification

Documents required

  • Application for equivalency determination
  • Proof of identity (passport / identity card)
  • Approval / professional permit
  • curriculum vitae
  • Tabular listing of the advanced training and professional experience
  • Proof of further training and professional experience (further training certificate, certificate, seminar certificate, project work, practical tasks)
  • If applicable, certificate of conformity / proof of activity over the last five years
  • If applicable, evidence from another EU member state, EEA or contracting state that a further training course in a third country has been credited
If the documents and certificates are not issued in German, they must also be submitted in a certified translation by a publicly appointed or sworn translator or interpreter.
