Process guides support you as a young person with (threatened) disability or as a family member in realising the benefits to which you are entitled.

Detailed description

If you are a young person with a (threatened) disability or a family member of a young person with a (threatened) disability, you may need support from various agencies or want to make use of services such as social or rehabilitation services. Knowing which agency is responsible for which issue and what rights you have is often difficult to understand.

The process pilots can help you here. The process pilots

  • know the responsible authorities and put you in touch with the appropriate contact persons.
  • help institutions to work together.
  • accompany you to appointments and planning procedures upon request.
  • are committed to ensuring that your individual participation and support needs are met.

The process guides adapt to your wishes and support you in finding the help you need. The advice offered by the process guides is independent and confidential. The process guides only take action if you want them to. The process guides deal with your concerns individually and flexibly and try to find the right offers for your personal life situation together with you.

If necessary, you can arrange a consultation with the process guides. The consultation can take place in the consultation rooms in the social welfare office or at cooperating institutions. If you wish, the process guides can also come to your home or advise you online.

Prerequisites

  • You are under 27 years of age and (possibly) at risk of or affected by a disability or
  • You are in one of the following relationships with the person named above:
    • You are a family member or a person with parental responsibility or custody
    • You are a carer or have another parental authority
    • You are the legally appointed carer

The Child and Youth Strengthening Act, which came into force in June 2021, set the course for inclusive child and youth welfare. The aim of the law is that from 2028 onwards, the integration assistance services for all children and young people with disabilities will be brought together under the umbrella of child and youth welfare, the so-called inclusive solution or help from a single source. The process pilots support this long-term process by using their experience from the advisory contacts with the target group to derive recommendations for inclusive organizational development.

Procedure

  • You contact the process guides by email or telephone and state your concern.
  • If necessary, the process guides will help you immediately. If necessary, you can arrange a consultation appointment with the process guides.
  • You attend the consultation appointment. If necessary, a person can attend the appointment to provide sign language interpretation or translation into your native language.
  • The process pilots will work with you to find a solution to your problem. If necessary, they will support those who are not involved in the process in implementing it.
