Detailed description
If you are a young person with a (threatened) disability or a family member of a young person with a (threatened) disability, you may need support from various agencies or want to make use of services such as social or rehabilitation services. Knowing which agency is responsible for which issue and what rights you have is often difficult to understand.
The process pilots can help you here. The process pilots
- know the responsible authorities and put you in touch with the appropriate contact persons.
- help institutions to work together.
- accompany you to appointments and planning procedures upon request.
- are committed to ensuring that your individual participation and support needs are met.
The process guides adapt to your wishes and support you in finding the help you need. The advice offered by the process guides is independent and confidential. The process guides only take action if you want them to. The process guides deal with your concerns individually and flexibly and try to find the right offers for your personal life situation together with you.
If necessary, you can arrange a consultation with the process guides. The consultation can take place in the consultation rooms in the social welfare office or at cooperating institutions. If you wish, the process guides can also come to your home or advise you online.
