Using the paper combination form, parents can apply for various administrative services related to the birth of their child. This service is available for births in Hamburg maternity clinics, the Hamburg Birth Center, and the Center for Birth and Health.

Detailed description

With the paper combination form "Child benefit made easy" you can:

  • determine your child's name,
  • Apply for child benefit,
  • Order and pay for additional birth certificates.

As soon as the birth is registered with the registry office, your child will be automatically registered in the residents' register. At the same time, your child's tax identification number will be applied for at the Federal Central Tax Office.


After processing, you will receive by mail:

  • the birth certificates,
  • your child’s tax identification number,
  • the child benefit notice.

Child benefit is transferred directly to your bank account.


You can use this service for births in all Hamburg maternity hospitals, the Hamburg Birth Center, or the House for Birth and Health. The combined form is available online as a PDF or on-site in paper form.

 

Information

Prerequisites

The combined form service is available to all parents who give birth in a Hamburg maternity clinic, the Hamburg Birth Center, or the House for Birth and Health. They do not need to be registered in Hamburg.

Documents required

Please attach the following documents in original, unless otherwise stated.


Foreign-language documents must be submitted along with a German translation prepared by a sworn or recognized translator. You will receive your originals back along with your child's birth certificates.

  • For married couples: marriage certificate with reference to the registration of the spouses' births (if this reference is missing, the spouses' birth certificates must also be submitted); if applicable, a certified copy of the family register (if married before 2009).
  • For unmarried parents: mother's birth certificate. For the father's registration: father's birth certificate and the certificate of acknowledgment of paternity. If the parents have joint custody, also attach the certificate of the custody declaration. If available, also submit the certificate of naming.
  • If the mother is divorced or widowed: marriage certificate with information about the dissolution of the marriage. If this information is missing, the divorce decree or death certificate must also be submitted.

The registry office may request the submission of additional documents if this is necessary to verify information.

Please note

In the case of twin or multiple births, please complete a separate combined form for each child.

Deadlines

Your child's name must be decided within four weeks of birth at the latest.

Procedure

1. Receive and complete the form

  • You can find the combined form online to complete and print. Alternatively, you can pick it up in paper form at your Hamburg maternity clinic, the Hamburg Birth Center, or the House for Birth and Health.
  • Please complete the form completely, sign pages 2 and 3, and attach the required documents.
  • Place everything in the envelope you received at your birthing facility.
  • After the birth, please return the filled envelope there.
  • Please transfer the fees for ordered birth certificates to the account specified in the information sheet of your birthing facility and on page 3 of the information sheet "Information on the Service".

2. Forwarding to the registry office

  • Your birth facility will forward the envelope and birth notification to the responsible registry office.
  • The registry office officially registers the birth and issues the requested birth certificates. You will receive these by mail.

3. Residents’ register and tax identification number

  • Your child will be registered in the residents’ register.
  • The data is also transmitted to the Statistical Office for Hamburg and Schleswig-Holstein.
  • The residents' registration office will inform the Federal Central Tax Office. They will create your child's tax identification number and send it to you by mail.

4. Child benefit

  • The registry office forwards your child benefit application directly to the family benefits office.
  • The family allowance office automatically receives your child’s tax identification number.
  • As soon as all data is available, you will receive the child benefit notice by mail.
  • Child benefit will be transferred directly to your specified account.

Processing time

You will usually receive the requested benefits by mail within approximately 10 days, provided you have submitted all the necessary documents for issuing your birth certificate and applying for child benefit. If any documents or information are missing, the responsible institution will contact you.

Fees


  • Birth certificates for parental allowance and maternity benefits are provided free of charge.

  • You can order additional birth certificates for your personal needs (e.g. for kindergarten, baptism, application for ID card or passport) using the combination form – these are subject to a fee.
