If your license certificate is lost or destroyed, you can apply for a replacement certificate.

Detailed description

If the original of your license certificate has been destroyed or lost, you can apply for a duplicate copy (replacement certificate).

Prerequisites

  • You have already been issued a certificate of approval, which you have lost, for example, due to loss or theft.

Documents required

The responsible authority will inform you accordingly upon your request.

Please note

When the replacement certificate is issued, the original certificate is declared invalid. If you recover the original certificate of approval, you must return it to the responsible authority.

  • The responsible authority will examine your application and, if necessary, request any missing information or documents from you.
